Recovery of a misinserted gastrostomy tube during replacement: effectiveness of gastropexy using a 'Funada style' kit.

چکیده
reported the efficacy of gastropexy using a ‘Funada style’ kit [1, 2] . Our patient, with a misinserted gastrostomy tube, was successfully treated during replacement using the gastropexy technique. Dear Sir, Percutaneous endoscopic gastrostomy (PEG) is a widely accepted method of providing long-term nutrition. However, as this procedure becomes more common for nutritional support, numerous complications have been described. We describe a case in which a gastrostomy tube was misinserted during tube replacement; endoscopic gastropexy successfully prevented peritonitis in this patient. A 65-year-old man was admitted for replacement of a gastrostomy tube, which had been inserted after an operation for neck cancer. Following tube replacement, a gastrografin study was performed through the gastrostomy tube; this study revealed that the tube had been misinserted. We immediately fixed the gastric wall to the abdominal wall using the gastropexy technique ( fig. 1 ). The patient showed no symptoms suggesting peritonitis. Gastropexy involves a technique in which the anterior gastric wall is non-surgically sutured to the abdominal wall. The procedure is often performed prior to the placement of a tube, and some studies have Published online: August 8, 2007

Percutaneous endoscopic gastrostomy with Funada-style gastropexy greatly reduces the risk of peristomal infection
BACKGROUND AND AIMS Peristomal wound infections are common complications of percutaneous endoscopic gastrostomy (PEG). The Funada-style gastropexy device has two parallel needles with a wire loop and suture thread, and was developed about 20 years ago in Japan. This kit has allowed us to perform dual gastropexy very easily; PEG with gastropexy has become a very popular technique in Japan. The p...

BACKGROUND/AIMS To determine the prevalence and time-course of t-fastener migration after gastropexy deployment. METHODS We reviewed our procedural database for all percutaneous gastrostomy and gastrojejunostomy tube insertions performed over a 14-month period using a widely accepted t-fastener kit for gastropexy (Kimberly-Clark). Of 201 patients, 71 (41 males, 30 females; mean age, 56 years)...

CONTEXT Enteral feeding is indicated for patients unable to maintain appropriate oral intake, and percutaneous endoscopic gastrostomy (PEG) is the most adequate long-term enteral access. Peristomal infections are the most common complications of PEG, occurring in up to 8% of patients, despite the use of prophylactic antibiotics. The "introducer" PEG-gastropexy technique avoids PEG tube passage ...

Percutaneous endoscopic colostomy (PEC) has been established as an alternative method of intestinal decompression and irrigation for chronic intestinal pseudoobstruction, relapsing sigmoid volvulus, neurogenic bowel, and constipation [1]. However, the thin, distended colonic wall can easily be torn during creation of the PEC with the pull method, and inadvertent traction of the PEC tube and col...
